Specifications include, but are not limited to: respond to Work Requests in writing, including providing a Cost Estimate; provide Services for DHHS as approved in accordance with section 5.1(2); not begin work for any Work Request prior to receiving written approval from DHHS; conduct Services in accordance with the guidelines issued by the Federal Office of Management and Budget, the U.S. Government Accountability Office, DHHS, and the American Institute of Certified Public Accountants; submit the draft Deliverables for Services as approved for each Work Request, as directed by the DHHS. Audit/review work papers for each health care facility, program or agency must be submitted for review if requested by DHHS; incorporate feedback received from the DHHS on draft Deliverables and submit final Deliverables to the DHHS; immediately notify the DHHS in writing when the Contractor considers the records of a particular health care facility, program or agency are not adequate or are unavailable to complete Services in accordance with the approval given in section 5.1(2) and identify the deficiency or deficiencies. The following guidelines apply: A. If the DHHS determines that the deficiencies cannot be corrected by the institution or agency, the Contractor shall immediately discontinue the Services and prepare a report on its findings; and B. Billing for Services not completed because of inadequate records shall not exceed 10 hours, or actual time spent, whichever is less, unless the DHHS grants written approval. identify management and staffing changes from the personnel listed in the proposal and ensure that replacement management and/or staffing have similar qualifications; and maintain a business office within the State of Utah and have regular business hours Monday through Friday excluding state holidays.
