Specifications include, but are not limited to: Friendships unlimited will provide an array of peer support, psychosocial rehabilitation, social skills, and recreational opportunities to individuals experiencing behavioral health conditions. Individuals will be Waukesha County residents diagnosed with a behavioral health condition and may or may not bereceiving services directly from Waukesha County DHHS. Individuals attending the recovery center will bescreened by the program for eligibility, the program will maintain a roster of all participants, and willengage in active outreach to inactive participants. The program will be responsible for developing a dailyschedule of activities to promote recovery, well-being, and social/community belonging for participants.The program will be required to maintain documentation of services and data reporting to Waukesha County DHHS as part of program operation. Applicants to this RFP are encouraged to propose additional services that applicant believes would be beneficial to participants. Friendships Unlimited is an authorized provider within the Waukesha County Comprehensive Community Services (CCS) and Community Support Program (CSP) network and will offer an array of Medicaidbillable individual or group services as authorized within the participant’s recovery plan. Referral ofindividuals to Friendships Unlimited services as part of a CCS or CSP recovery plan will be made by theindividual’s Waukesha County DHHS service facilitator. Unless identified, the components listed beloware defined by the Wisconsin Forward Health Handbook as part of the CCS service array. Services authorized for individuals referred to Friendships as part of their CCS recovery plan must fall within the descriptions below, as identified in the individual’s DHHS authorized recovery plan. These descriptions also will be applicable to individuals referred as part of a CSP recovery plan. Additionally, these services would be appropriate for all Friendships Unlimited participants
