Specifications include, but are not limited to: The Louisville Metro Department of Public Health & Wellness (LMPHW) requires the services of a certified medical laboratory facility to perform various standard laboratory procedures on an as needed basis. Laboratories must meet the following specifications: 1. Be accredited and licensed by the U.S. Center for Medicare and Medicaid Services. Be Clinical Laboratory Improvement Amendment of 1988 (CLIA) certified. Attach copy of accreditations and licenses to bid. 2. Provide afternoon specimen pick-up services at each clinic site on a daily Monday – Friday or as-needed basis. A listing of the clinic sitesis provided below. LMPHW reserves the right to add or delete clinic sites as it may deem necessary. 1. Accounts will be established for each of the clinic locations. Monthly billing will be by account numbers. Some billing will be done by the reference laboratory to the patient orthird party. This will be determined by the clinic site of the specimen. Bid prices will apply to all payers. The billing information will be provided to the reference laboratory by LMPHW. 2. Billing will be based on a 30-day, month in arrears. Vendor shall send invoices directly to Accounts Payable at invoices.omb@louisvilleky.gov 3. The vendor shall be required to submit monthlymanagement reports to OMB on the spending cost associated with the contract. All reports are to be in electronic format. Information shall include, but is not limited to, all LMPHW locations serviced and a listing of any billing orservice issues. Additional reports may be required upon request. All such information shall be available within one (1) week of request. 4. The vendor must provide a single point of contact to serve as the Account Manager for the contract. The name, telephone number, and e-mail address of the Account Manager shall be provided to the Contract Administrator at the time of contract award. Contractor must provide written notification of any change in the Account Manager. Response time to customer service inquiries should be no more than 24 hours. 5. Vendor must have the ability to interface with LMPHW Electronic Health Record 6. Provide at no additional cost, all required testing materials, including: tubes, needles, plastic transport bags, specimen containers, centrifuges, specimen pick up boxes. 7. Provide in-service training as needed, on specimen collection and handling 8. Comply with Health Insurance Portability and Accountability Act (HIPAA) requirements and be willing to sign a Business Associate Agreement (BAA) with LMPHW. Comply with Code of Federal Regulations requirements for confidentiality of substance use disorder patient records. 9.
