Specifications include, but are not limited to: provide personalized, in-home support for pregnant persons and families with children birth to age 2 who face risk factors through the implementation of the Nurse Family Partnership model specifically focused on support for firsttime, low-income pregnant persons. Regular home visits are administered by trained personnel who provide individualized support to build parents’ knowledge, skills, and attitudes related to parenting, as well as enhance their own individual and family well-being. The implementation of the Nurse Family Partnership model will expand the continuum of home visitation models available to families in the region. Implementation of the strategy requires the implementation of an approved evidence-based home visitation model that includes the following main elements: ● individual home visits ● child developmental and equipment-based sensory screening ● family functioning, parenting skills assessments and mental health screenings ● resources and referrals
