The scope of services proposed involves a Commercial Off the Shelf (“COTS”) Immunization Information System in a format as has been implemented by other CDC Immunization Awardees. Hawaii seeks an IIS that meets the full CDC IIS Functional Standards (refer to: https://www.cdc.gov/vaccines/programs/iis/funcstds.html). The responder should describe the services, project plan, and technical details for implementation of an IIS for Hawaii, similar to deployment in another local health jurisdiction. The responder should propose associated project staffing for completion of all technical and implementation tasks and describe prior experience and qualifications. The responder should propose estimated costs associated with a workplan for successful configuration, deployment, training, and system use over an approximate sixteen-to-eighteen-month timeline.
