Specifications include, but are not limited to: The servicesto be provided by the vendorshould be broken down into the following categories and subparts: • SpecimenCollection Services(It isrequired that allsamples be taken on the employer’s work sites from a Mobile Testing Unit. • Laboratory Services • Medical Review Officer (MRO) Services • Record keeping and Data Collection • The contractor shall perform on-site drug and/or alcohol collections for state employees via the Mobile Testing Unit. Each collector will arrive at the (state) designated on-site testing location at least ten (10) minutes prior to the actual scheduled testing time. All collectors are to carry a valid driver’s license and company identification. The collector will have collection kits, Custody Control Forms (CCF) and Alcohol Testing Forms (ATF’s) at each on-site testing event (Scheduled or Emergency). Listings of Agency Site Testing Locations, Facility Locations, Work Locations of CDL Drivers, and Contact Information is attached as a downloadable .zip file. • The Mobile Testing Unit shall meet, or exceed, the following required specifications: Power: AC/DC, Generator equipped HVAC: Electric Heat and Air Conditioning (No propane tanks) Restroom: No drop ceiling, lock box in restroom, water in toilet (bluing), no sink in restroom (sink to be outside restroom area), minimum open floor space of restroom 35’x50’, no windows in restroom. Testing Area: Sink to wash hands, file system to keep Custody & Control Forms (CCF’s) for all testing locations onboard, storage for testing supplies, two (2) breath alcohol testers, privacy for donor, seating (not in the cab) for donor. Outside Testing Unit: Unmarked, no window in testing area.
