Please note: This is a Sole Source Notification. Specifications include, but are not limited to: 1. Maintain status acting as an insured, licensed physician in the State of Utah. a. Act as a consultant to the Health Department on medical and clinical issues such as immunizations, sexually transmitted diseases, tuberculosis, and other communicable diseases. b. Consult on the development of clinical and medical protocols for Health Department programs. c. Provide consultation and guidance in the event of an emergency or other disaster. d. Provide consultation to the Environmental Health Department and Emergency Response Coordinator as necessary. e. Services to be provided at the Health Department’s clinics, the Physician’s office, via telephone or other electronic means, or at a designation mutually agreed upon by both parties.
