Specifications include, but are not limited to: The County of Hennepin, State of Minnesota (“County”) is soliciting proposals for cardiovascular anddiabetes health support services for Hennepin County Public Health Promotion (PHP). The Hennepin County Board of Commissioners has approved up to $100,000.00 to fund this Request for Proposals (RFP),which authorizes PHP to support cardiovascular and diabetes health among Hennepin County residents, especially disproportionately impacted African American/Black and Indigenous women, primarily 40 years of age and older. This RFP will fund community organizations to improve heart health among African American/Black and/or Indigenous women in Hennepin County through increased social-physical activities, cultural activities, group and peer support, and/or lifestyle changes. PHP anticipates awarding up to 10 contracts from this RFP, ranging from $10,000.00 to $50,000.00 each.
