Specifications include, but are not limited to: a. Maintain individual files, which document: • Proof of Louisiana Residency • Proof of Income • Proof of HIV Diagnosis • Client Demographics • Services Provided • Plan of Care/Follow up b. Conform to the reporting requirements of the Ryan White Services Report (RSR) by implementation and use of the Ryan White CAREWare database, with timely submission of electronic data and invoices on a monthly basis. c. Be knowledgeable of relevant statewide and local resources, in each parish of their coverage area, to maximize available services and access for eligible clients. d. Be available to provide services to eligible clients through telephone contact, in-person visits with the client in an appropriate and clientcentered setting or in the offices of the subrecipient organization.
