Specifications include, but are not limited to: 1) Maple Leaf Drive (IVO 3308 Maple Leaf Drive): a. Remove approximately 30-LF of 36-inch CMP. Replace with 30-LF of dual 36- inch diameter RCP or HDPE pipe (this shall include tree/stump removal, if necessary; excavation and removal of existing culvert; installation of new cross culverts; road embankment repair, culvert backfill, and pavement restoration in accordance with KYTC standards; finish grade and stabilization; and disposal of excess spoils). b. Install two (2) dual 36-inch winged headwalls. One at the inlet and outlet end of the pipe. (Given the skewed nature of the crossroad culvert in this location, Oldham County will accept the following alternatives for headwall installation: 1) standard headwall joined to a mitered pipe or, 2) skewed headwall joined to a perpendicular pipe. For headwalls refer to KYTC standard drawings RDH-510- 04 and RDH-530-04). The outlet headwall shall be constructed with energy dissipaters. c. Regrade and stabilize 15LF within the drainage channel at the inlet headwall to obtain positive drainage and connectivity with the upstream area (This shall include any tree/stump removal, excavation and removal of spoils, finish grade and stabilization; and disposal of excess spoils). d. Install and maintain appropriate Erosion Protection and Sediment Control (EPSC) Best Management Practices (BMPs) during construction. 2) Maple Leaf Drive (IVO 3200-3206 Maple Leaf Drive): a. Remove approximately 30-LF of dual 24-inch CMP. Replace with 30-LF of dual 36-inch diameter RCP or HDPE pipe (this shall include tree/stump removal, if necessary; excavation and removal of existing culvert; installation of new cross culverts; road embankment repair, culvert backfill, and pavement restoration in accordance with KYTC standards; finish grade and stabilization; and disposal of excess spoils). b. Install two (2) dual 36-inch winged headwalls. One at the inlet and outlet end of the pipe. For headwalls refer to KYTC standard drawings RDH-510-04 and RDH530-04. The outlet headwall shall be constructed with energy dissipaters. c. Install and maintain appropriate Erosion Protection and Sediment Control (EPSC) Best Management Practices (BMPs) during construction. 3) Maple Leaf Drive (IVO 3403 Maple Leaf Drive): a. Remove approximately 30-LF of 36-inch CMP. Replace with 30-LF of 36-inch diameter RCP or HDPE pipe (this shall include tree/stump removal, if necessary; excavation and removal of existing culvert; installation of new cross culverts; road embankment repair, culvert backfill, and pavement restoration in accordance with KYTC standards; finish grade and stabilization; and disposal of excess spoils). b. Install two (2) 36-inch winged headwalls. One at the inlet and outlet end of the pipe. For headwalls refer to KYTC standard drawings RDH-110-02. The outlet headwall shall be constructed with energy dissipaters. c. Install and maintain appropriate Erosion Protection and Sediment Control (EPSC) Best Management Practices (BMPs) during construction. 4) Forrest Park Drive (IVO 2103 Forrest Park Drive): a. Remove approximately 30-LF of 15-inch CMP. Replace with 30-LF of 15-inch diameter RCP or HDPE pipe (this shall include tree/stump removal, if necessary; excavation and removal of existing culvert; installation of new cross culvert; road embankment repair, culvert backfill, and pavement restoration in accordance with KYTC standards; finish grade and stabilization; and disposal of excess spoils). b. Install two (2) straight 15-inch headwalls. One at the inlet and outlet end of the pipe. c. Install splash pad (Class II Riprap, 05-LF wide by 08-LF long, 1.5-feet deep overtop of geotextile fabric) within the drainage channel at the outlet headwall (This shall include any tree/stump removal, excavation and removal of spoils, finish grade and stabilization; and disposal of excess spoils). d. Install and maintain appropriate Erosion Protection and Sediment Control (EPSC) Best Management Practices (BMPs) during construction. 5) Running Brook Road (IVO 2312 Running Brook Road): a. Remove approximately 30-LF of dual 42-inch CMP. Replace with dual 30-LF of dual 4-foot x 3-foot concrete box culvert (this shall include tree/stump removal, if necessary; excavation and removal of existing culvert; installation of new cross culvert; road embankment repair, culvert backfill, and pavement restoration in accordance with KYTC standards; finish grade and stabilization; and disposal of excess spoils) Note: Box culverts will be furnished by the Oldham County Road Department. Bidder shall be responsible for loading and delivering box culverts from the Road Department to construction site. b. Install and maintain appropriate Erosion Protection and Sediment Control (EPSC) Best Management Practices (BMPs) during construction 6) Mohawk Drive (IVO 3703 Mohawk Drive): a. Remove approximately 50-LF of 42-inch CMP. Replace with 50-LF of 42-inch diameter RCP or HDPE pipe (this shall include tree/stump removal, if necessary; excavation and removal of existing culvert; installation of new cross culvert; road embankment repair, culvert backfill, and pavement restoration in accordance with KYTC standards; finish grade and stabilization; and disposal of excess spoils). b. Install two (2) straight 42-inch headwalls. One at the inlet and outlet end of the pipe. c.
