Specifications include, but are not limited to: Jackson County recognizes that it has been several years since these dollars were put out for community bidders and that many of our community needs and areas of focus have shifted. Jackson County Health and Human Services, at the direction of the Board of Commissioners and the Budget Committee, using the 2023 Community Health Assessment as a guide, is releasing this Request for Proposal for the following key areas: Priorities & Strategies - Priorities and strategies are the activities designed to achieve the outcomes. For example, by increasing outreach programs for the elderly experiencing or at risk of experiencing loneliness (priority) through expansion of food distribution and regular volunteer outreach to this population at least weekly (strategy); we hope to decrease loneliness and isolation experienced by the older adult population, reducing the risk of suicide and other poor health outcomes. Specific priorities and strategies have been selected for each high-level outcome (see attachments). Intermediate Outcome - The outcome is the interim change which your project wants to make in the population served. It must be measurable and it must focus on client change not the service given. For example, school attendance will increase for 75% of youth involved in the identified mentoring project. Specific outcomes and measurement tools have been selected for each strategy and are already written into your required logic models. (see Outcome Information Packets) Outputs - The output refers to the number and kinds of services which your project will provide. Outputs can be counted. Logic Model - Logic Models must be completed for all proposals and will need to include your project strategy, resources, activities, outputs, and intermediate outcome(s). Please use the logic model form which is specific to the outcome you are addressing.
