Specifications include, but are not limited to: The purpose of this RFP is to select the best-qualified proposer (hereinafter referred to as “Vendor) and award a County-approved contract to perform the Services and to satisfactorily complete all activities associated with the Goods/Services. The applicant will identify which Pillar will be addressed with the funding – Pillar Two, or Pillar Four or both. The priority population for Pillar Two is people who are newly diagnosed, not engaged in care or not virally suppressed with a focus on 15- 34 year olds. Activities can include but are not limited to: • Outreach • Social media • Youth apps • Support groups • Partnering medical services with support services, substance abuse or mental health services • Housing • Transportation • Mobile units • Street medicine • Substance abuse care and treatment • Mental health care and treatment • Telehealth • Community health workers • Community engagement specialists
