Specifications include, but are not limited to: 1. Provide a case coordinator to oversee the medical evaluation program. Provide name, phone number, email address and all relevant contact information for the case coordinator. 2. Schedule appointments for medical evaluations within 15 days of the request by the Commissioner. 3. Provide medical evaluation reports and findings to DWC within 15 calendar days of the completed evaluation. 4. Immediately notify DWC if worker has been seen or evaluated by Medical Evaluator at an earlier date, providing the date of the earlier evaluation. 5. Conduct the medical evaluations in accordance with the American Medical Association (AMA)Guidelines and other applicable standards. 6. Provide medical evaluations reports, including a completed copy of the latest International Labor Office (ILO) classification report, and findings to DWC within 15 calendar days of the completed evaluation. 7. Maintain current National Institute for Occupational Safety and Health (NIOSH) “B-Reader” certification and current board certification as a pulmonary specialist within the Commonwealth of Kentucky. 8. Within fifteen days of the DWC’s request, provide original chest x-rays or digit x-rays taken pursuant to prior KRS 342.315 or 342.316 evaluations. 9. Be available for depositions when necessary. If travel is required for a deposition, travel expenses will be reimbursed pursuant to the regulations of Employees’ Reimbursement for Travel 200 KAR 002 006, Title 200 Chapter 2 Regulation 006 • Kentucky Administrative Regulations • Legislative Research Commission Either original or certified copies of receipts shall be submitted for airline tickets, hotel bills, rental car charges and any other miscellaneous expenses. Lodging shall be the most economical, determined by considering location of the lodging. Commercial airline travel shall be the lowest negotiated, coach or tourist. Additional expense for first-class travel shall not be reimbursed by the state. Total travel reimbursement amount for the length of the contract shall not be included within the total cost of the contract.
