Specifications include, but are not limited to: 3.1 Be qualified to administer and guide the ASPHL personnel to ensure the performance of high complexity tests and shall be eligible to be an operator of a laboratory within the tasks and requirements listed herein and per CLIA §493.1443 (link below). https://www.ecfr.gov/current/title-42/chapter-IV/subchapter-G/part493#493.1443 3.2 Possess a current license as a laboratory director issued by the State of Arizona, if such licensing is required. 3.3 Be a doctor of medicine or doctor of osteopathy licensed to practice medicine or osteopathy in the State of Arizona and be certified in anatomic and/or clinical pathology, by the American Board of Pathology or the American Osteopathic Board of Pathology or possess qualifications that are equivalent to those required for such certification. 3.4 Be a doctor of medicine, a doctor of osteopathy or doctor of podiatric medicine licensed to practice medicine, osteopathy or podiatry in the State of Arizona and have at least one (1) year of laboratory training during medical residency (for example, physicians certified either in hematology and medical oncology by the American Board of Internal Medicine or have at least two (2) years of experience directing or supervising high complexity testing. 3.5 Hold an earned doctoral degree in a chemical, physical, biological or clinical laboratory science from an accredited institution and have and keep current certification by a board approved by Health and Human Services (HHS).
