Specifications include, but are not limited to: The purpose of this Request for Proposals (RFP) is to solicit proposals to enter into a contract with a qualified vendor to obtain fully-insured health insurance coverage and related services for eligible students of the University of Utah, hereafter referred to as the "University," and their eligible dependents. All full-time degree-seeking international students with F1 or J1 visas are automatically enrolled in these insurance plans, unless proof of comparable coverage is furnished. Eligibility for these plans is limited to: Subsidized Graduate Students on a contributory basis (students meeting the internal qualifications established by The University of Utah to be eligible for the Subsidized Graduate Plan, including Research and Teaching Assistants. Refer to the following link for more details: https://gradschool.utah.edu/funding/tbp/gship/index.php ); Graduate Students on a voluntary basis (eligible if enrolled for three (3) or more credit hours during 2 semesters each academic year); and Undergraduate Students on a voluntary basis (eligible if enrolled for six (6) or m ore credit hours during 2 semesters each academic year). Eligible students who enroll may also insure their eligible dependents. Eligible Dependents include the student’s legal spouse and children under 26 years of age. The companies submitting proposals in response to this RFP will hereafter be referred to as “Vendor.”The University is examining several alternatives of providing this insurance coverage and may decide, after reviewing proposals submitted, not to enter into any agreement.
