Specifications include, but are not limited to: The Community Based Group Home and Support Services shall 1. Provide children with 24/7 safe and temporary living environment; 2. Provide Community Based Group Home (CBGH) in West Hawaii to allow children to remain in their respective community; 3. Provide Community Based Group Homes (CBGH) in West Hawaii to allow children to continue in their school of origin; 4. Provide crisis stabilization, behavioral supports, and on-going counseling to children in care. 5. Assess each child upon placement, identify their needs and work with DHS to make referrals to meet those needs; 6. Provide knowledgeable trained staff to stabilize and support the needs of the child using a trauma informed care approach; and 7. Provide support to the child’s parent/caregiver to assist in the transition from the Community based Homes to the child’s new living setting. Service Objectives: a. Prevent or reduce the number of runaways; b. Improve placement stability; c. Improve academic outcomes; and d. Improve the well-being for age 12-17 youth (e.g., behavioral and emotional functioning, social functioning, school attendance, physical health and development, mental health, pro-social activities).
