Specifications include, but are not limited to: Part 1: Respond to this RFP and, through the competitive process achieve both the minimum thresholds on both the program and fiscal requirements and • For Family Care, have one of the highest total cumulative scores. DHS intends to pursue Contract negotiations with at least two, and up to four, Vendors for the geographic service region. • For Family Care Partnership, have one of the highest total cumulative scores. DHS intends to pursue Contract negotiations at least one, and up to three Vendors for the geographic service region. Reference Sections 1.4 and 8.2 for information about Contracts for the service regions and scoring criteria. Part 2: Achieve certification through DHS by having met all requirements including, but not limited to, applicable federal and state statutes and DHS Administrative Code sections. The Vendor is expected to meet all certification requirements no later than 45 days prior to the Contract’s effective date. Part 3: Contract with DHS to deliver specific managed long-term care benefits (Section 1.2.5) to specific eligible populations (Section 1.2.6) in a designated geographic service region(s) for the Family Care Program or Family Care Partnership Program. (Section 2.5). The Department of Health Services does not guarantee to purchase any specific quantity or pay any minimum Contract price during the term of the Contract. Proposals that require a minimum number of commodities or services be ordered shall be disqualified.
