Specifications include, but are not limited to: a. Issue initial notification letters to newly eligible employees and dependents b. Generate formal benefit continuation offers c. Generate and distribute continuation election forms d. Issue premium billing notices e. Issue late premium due notifications f. Collect premiums g. Issue open enrollment changes h. Issue cancellation notices i. Issue conversion coverage notices, where applicable j. Reconcile payments to health, dental, vision, life, FSA, STD, LTD vendors k. Maintain records of all program data l. Production of Form 1095-B i. COBRA/continuee enrollments ii. HCSP enrollment based on information provided by the county m. Administer appeals in collaboration with the county. n. The county offers the following benefit plans that would be required for set up and administration: 1. Health Insurance i.
