Specifications include, but are not limited to: This RFP will provide CCM services to the entire priority population, including all eligibility categories defined in section 4.11. CCM services shall be provided at priority populations’ residences, at various locations in the community, and/or sites specified in their support plan throughout Hennepin County and out of county locations. Inter-county waiver case management services will be provided to the priority population residing in Hennepin County, whose care is primarily the financial responsibility of another county and to persons residing out of county, whose care is primarily the financial responsibility of Hennepin County. Proposers must have the capability of providing service to persons residing in counties throughout the state. Proposers should demonstrate ability to serve persons of diverse cultures and/or languages. CCM services assist the priority population to access needed services, including medical, social, educational, and other services regardless of the funding source. CCM activities include but are not limited to: • Assisting the person to identify potential service providers; • Assisting the person to maintain eligibility and access services; • Coordination and monitoring of services; • Evaluation and monitoring of the services identified in the support plan; • Informing the person or the person’s legal guardian, conservator, or parent (if the person is a minor) of service options; • Development of the support plan within the county approved allocation and based on a person’s assessed needs;
