Specifications include, but are not limited to: CCHHS will look to the contractor to execute the required tasks effectively to result in the provision of comprehensive behavioral health services for the Pathways to Community Program, serving individuals who are released from the Coconino County Detention Facility. The Independent Contractor will have staff physically present in the PTC office space adjacent to the Detention Facility, with the ability to coordinate behavioral health services Monday thru Friday 8-5pm. These services shall include: 7 - Clinical services on site to include intake/enrollment with local behavioral health homes/providers as appropriate, ASAM, connection up to and including reengagement into outpatient services and behavioral health/psychiatric medication appointments as appropriate, connection to MAT/MOUD induction and continuation as appropriate. - Clinical staff on site may provide crisis triage support as needed and when appropriate. - Facilitate linkages to care to support clients in accessing services, including completion of warm handoff to community partners in partnership with Pathways staff as appropriate for behavioral health care coordination, regardless of health plan enrollment (AHHHCS/Medicaid/Private). For example, securing bed dates for residential treatment, connection to psychiatric inpatient services, coordination with the Tribal Regional Behavioral Health Authority, etc.
