Please note: This is a Sole Source Notification. Specifications include, but are not limited to: 4.1 Learning Collaborative. The Contractor shall: (1) create a training curriculum for healthcare providers focused on how to identify patients with arthritis, conduct PAVS, provide counseling to encourage physical activity, and refer the individuals physical activity and the evidence-based healthy aging programs (https://healthyaging.utah.gov/find-workshop/#/); (2) identify subject matter experts, evidence-based literature, and national resources from the Osteoarthritis Action Alliance, National Association of Chronic Disease Directors, and Centers for Disease Control and Prevention for the development and implementation of the training; (3) recruit four to 10 clinic teams that serve persons with arthritis or underserved areas (e.g. rural areas, community health clinics, or geographic areas with poorer health outcomes; see https://map.utah.healthyplacesindex.org/) to participate in the training; (4) assist clinics to track outcome and process measures; (5) provide guidance and data support to clinics on how to identify adults (18 and older) with arthritis and implement workflows to conducts PAVS, healthcare provider counseling, and referrals; (6) create and conduct pre- and post-training evaluations of clinic workflows and practices related to arthritis management, PAVS, counseling, and referrals in order to assess the impact of the learning collaborative; (7) attend monthly meetings with the Department; and (8) create a final report on activities described in this contract and submit it no later than June 20 of each year. 4.2 Living Well Coalition. The Contractor shall: attend quarterly Living Well Coalition meetings; and attend a Living Well Coalition project group. 4.3 Reporting. The Contractor shall submit progress reports no later than April 1 and June 20 of each year that includes information about the clinics recruited, progress through the learning collaborative, and estimated number of people reached.
