Specifications include, but are not limited to: The purpose of this contract is to provide the Arizona Department of Child Safety Healthy Families Arizona program with a referral process for community providers, specifically hospitals in Maricopa County. 2.2 The Contractor will manage, distribute and maintain data regarding referrals and consent forms from the hospitals to the Healthy Families programs in Maricopa County. The Contractor will streamline the referral process, enhance communication with the hospitals, and ensure that families receive assignments to Healthy Families and/or other services. 2.3 The Contractor will have established Memorandum of Agreements or Memorandum of Understandings with the hospitals in Maricopa County and will maintain those agreements in conjunction with their Healthy Families Arizona contract.
