Specifications include, but are not limited to: • The contractorshall perform on-site drug and/or alcohol collectionsforstate employees via the MobileTesting Unit. Each collector will arrive at the (state) designated on-site testing location at least ten (10) minutes prior to the actual scheduled testing time. All collectors are to carry a valid driver’s license and company identification. The collector will have collection kits, Custody Control Forms (CCF) and Alcohol Testing Forms (ATF’s) at each on-site testing event (Scheduled or Emergency). Listings of Agency SiteTesting Locations, Facility Locations, Work Locations of CDL Drivers, and ContactInformation is attached as a downloadable .zip file. • The MobileTesting Unitshall meet, or exceed, the following required specifications: Power: AC/DC, Generator equipped HVAC: Electric Heat and Air Conditioning (No propane tanks) Restroom: No drop ceiling, lock box in restroom, waterin toilet (bluing), no sink in restroom (sink to be outside restroom area), minimum open floor space of restroom 35’x50’, no windows in restroom. Testing Area: Sink to wash hands, file system to keep Custody & Control Forms (CCF’s)for all testing locations onboard,storage for testing supplies, two (2) breath alcohol testers, privacy for donor, seating (not in the cab) for donor. OutsideTestingUnit: Unmarked, nowindowin testing area. • Establish the capability to handle testing during periods when facilities may not be open. e.g. nights, weekends, holidays, etc. (24/7 TestingCapabilities) • Implement a collection process conforming to all applicable Federal and State requirements for urinalysis drug testing and applicable alcohol testing, including breathalyzer. • Establish a "chain of custody" procedure that complies with FederalRegulations. • Arrange for a courierservice for pickup and transfer ofsaid specimensto the federally approved laboratory. • Contract with a Federal Department of Health and Human Services' National Institute on Drug Abuse (NIDA) certified laboratory for testing. • Establish a procedure for random testing of employees on a quarterly basis. The number of alcohol and drug tests on an annual basis must meet, at least, the minimum requirements of Federal Regulations. Under Federal Regulations, a minimum of ten (10%) percent of employeesin eligibility pool must be tested annually for alcohol abuse. Under Federal Regulations, a minimum of fifty (50%) percent of employeesin eligibility pool must be tested annually for drug abuse. • Provide for a "chain of custody" in the collection and transfer ofsamplesto the NIDA "certified" laboratory and necessary forms relating to same.
