Specifications include, but are not limited to: 1) This project consists of areas, provided in pictures, and Google map, that have damaged concrete and subgrade. Concrete and subgrade shall be removed and replaced with compacted rock and rebar to allow for a new, 4000psi concrete slab of 8” in depth. The areas are indicated with pictures and measurements (See Exhibit A. As stated in Section 3.2, Prospective Bidders may conduct their own measurements at the Mandatory Pre-Bid Meeting. 2) One sunken catch basin shall be repaired and the area surrounding it shall be replaced with concrete per design requirements. 3) Barricade off the areas of work. 4) Call utility locates. 5) Obtain permits as required for driveway and sidewalk areas. 6) Saw cuts with water control. 7) Remove concrete. 8) Remove compromised base to 20” below grade to prepare for new base materials. 9) 12” of 3/4 minus crushed rock, compacted subbase. 10) Install #4 rebar or equivalent, 12” on center to concrete areas. 11) Pour 8” thick 4000psi concrete throughout. 12) Broom finished with tooled control joints. 13) Seal finished surface. 14) Remove all debris, clean the site each workday. 15) Haul clean materials to a local, appropriate disposal site. 16) Communicate with District staff as needed. 17) All work to be done in a professional, workman like manner. 18) All work to be done as per local codes requirements in effect at the time of installation. 19) All materials and equipment installed as per manufacturer’s instructions. 20) All areas shall be clean of debris around the building and parking areas before the project will be considered "complete". 21) The work is to be completed by the completion date shown on the Cover Page of this ITB.
