Specifications include, but are not limited to: The Office of Health Strategy (OHS) is seeking proposals from public and private organizations to work with the OHS, the Office of Early Childhood (OEC), and the Department of Social Services (DSS) to research and develop reimbursement and sustainability plans related to the Universal Nurse Home Visiting-Community Health WorkerPilot Program of Greater Bridgeport, known as Family Bridge.OHS is seeking innovative and creative sustainability and reimbursement plans that reflect a thorough understanding of the services to be provided, their intended impact, and their significance. The respondent’s proposed scope of work will include research, analysis,recommendation, development of a project plan and implementation assistance regardingcommercial and Medicaid reimbursement for the Family Bridge, inclusive of the UNHV andCHW components.
