Specifications include, but are not limited to: (a) Perform the following UBHPAC duties and ensure its staff: (1) Attend all monthly UBHPAC meetings; (2) Provide set up for in-person meeting locations, including teleconferencing capability; (3) Provide and covers the cost of drinks and a variety of light lunch food choices adequate for the expected number of in-person attendees. (4) Provide notifications, reminder emails, and electronic calendar invites to UBHPAC members prior to monthly meetings. Notifications and agendas from the UBHPAC executive committee must be posted as required by Utah Code Title 52, Chapter 4, the Utah Open and Public Meetings Act. (5) Manage applications for new UBHPAC members. This includes: (i) Maintaining a current list of UBHPAC membership and subcommittees. This list must include: (A) Name, address, email, phone number; (B) Membership status, (C) Represented organization, and (D) UBHPAC position, (ii) Maintaining an attendance record of the UBHPAC membership and subcommittees. (6) Record, prepare, and distribute meeting minutes to Department of Health and Human Services, Office of Substance Abuse and Mental Health (”DHHS”) program staff and all UBHPAC members via email and within 15 days of the meeting. Any audio recordings must be sent via email to DHHS program staff to be posted on the DHHS UBHPAC website. (7) Track meeting attendance and notify members who are not in compliance with meeting attendance requirements. (8) Provide orientation services to prepare new members. Orientation must include a manual with the UBHPAC history, role, and purpose. (9) Respond to all DHHS questions and inquiries received from the public and provide DHHS with current phone numbers, email addresses, physical addresses, and primary contact staff who shall be responsible to respond. (10) Coordinate to ensure the UBHPAC letter for the block grant submission is signed. (11) Contact agencies to recruit UBHPAC members that meet a profile required by the Substance Abuse and Mental Health Services Administration (12) Comply with all applicable local, city, county, and state licensing, accreditation, and certification requirements. (13) Ensure staff assigned to UBHPAC coordination do not serve as voting members of UBHPAC. The Contractor’s staff shall sign a conflict of interest statement. The Contractor shall provide this conflict of interest statement to DHHS within 30 days of the effective date of this contract. DHHS will provide the form to the Contractor if needed.
