Please note: This is a Sole Source Notification. Specifications include, but are not limited to: A. Circuits For circuits listed in Section II.A. or as added per this Section I.A., customer orders with a particular Service Term identified in the customer order which are made during the Contract Period will receive 5-year Service Term pricing, and any equipment costs associated with customer orders shall be spread over a 5-year term, even if the Contract Period will expire before the end of a 5-year Service Term. In the event that a new master contract for circuit products and services is not awarded and negotiated between DTS and CentraCom at the end of the Contract Period, DTS will pay the remainder of the equipment costs and all early termination fees in the form of a one-time payment at the end of the Contract Period. If a new master contract is awarded and negotiated between DTS and CentraCom, the equipment will continue to be billed through the remainder of the 5-year Service Term under the new contract according to the costs outlined in the original agreed upon quote and the terms agreed to under the new contract. Circuits under this Contract may only be disconnected through a written request sent to CentraCom by dtsprocurement@utah.gov or by authorized DTS personnel indicating the particular circuit and termination effective date. CentraCom will confirm receipt and will discontinue billing at the end of the month the circuit has been requested to be disconnected, subject to the early termination charges. Any removals must be confirmed, and vendor must CC dtsaccountspayable@utah.gov with the date the service will discontinue billing. Only the circuits identified in Section II.A and those which are added via the process in this section are subject to the fees indicated in the Contract. B. Communication Services For telephone, CentraCom and DTS acknowledge and agree that the communications services identified in Section II.B below are not subject to any particular Service Term or service level and are provided on a “best efforts” month-to-month basis. CentraCom and DTS further acknowledge and agree that the Services identified in Section II.B below are not subject to any particular contract pricing but may be governed by CentraCom’s tariff. Services under this Contract may only be disconnected through a written request sent to CentraCom by dtsprocurement@utah.gov or by authorized DTS personnel, indicating the particular service and termination effective date. CentraCom will confirm receipt and will discontinue billing at the end of the month the service has been requested to be disconnected.
