Specifications include, but are not limited to: The Connecticut Department of Public Health (DPH), Chronic Diseases Unit is seeking proposals from Connecticut public and private organizations and community-based agencies to provide training programs for the Alzheimer’s Disease and Related Dementias (ADRD) Program for medical professionals and community professionals. An anticipated total of approximately $100,000 annually for a total of $500,000 of federal Centers for Disease Control and Prevention (CDC) ADRD funds are available to support this project. o Component 1: Training for Medical Professionals will be supported by $50,000 annually for 5 funding periods and o Component 2: Training for Community Professionals will be supported by $50,000 annually for 5 funding periods.
