Specifications include, but are not limited to: The vendor is to provide all transportation, equipment and materials required to complete a gas leak and atmospheric corrosion survey for the City of Lebanon Gas System. Equipment shall include IR or RMLD unit(s), combustible gas indicator, probe rods, and any other necessary equipment. There are an estimated 317 miles of mains and 1100 services. The average service line length is approximately 100 linear feet. The system’s present unaccounted for is about 2%. The survey shall be conducted in accordance with 49 CFR Part 192.723. The survey will include approximately twenty-five percent (25%) of the existing distribution system, business districts, and public buildings. The complete survey shall include all transmission lines, distribution mains and service lines (service line extends from tap tee to outlet side of gas meter). Exact area or location of service areas to be tested will be determined by the City of Lebanon Gas Department and provided to the vendor upon start of survey. Any gas leaks found shall be located and logged on a written daily report form to be furnished by the vendor and approved by the City of Lebanon. A complete written log report shall include such items as classifications of gas leaks discovered, number of each classification, and the location of all gas leaks to the degree of determining whether the leak is on a service line or main. Above ground leaks, i.e., meter sets, regulator stations or any above ground piping shall also be shown on a written log report with a drawing or photograph detailing exact location of leak at time of inspection. All leak reports, above or below ground, shall have a unique numerical identification number. Each bidder shall furnish a minimum of three (3) job references of gas system surveys performed within the State of Tennessee or other states in the general region within the last five (5) years and any other documentation that will show qualification of consultant and/or employee performing actual leak survey work. Operator Qualification and PHSMA approved Drug and Alcohol plan documentation must be received and approved by the City of Lebanon Gas Manager before work can begin. The vendor shall maintain liability and comprehensive insurance in the minimum amount of $500,000.00, to cover their employees and operation of their equipment. Proof of insurance will be provided to the City of Lebanon by the vendor before survey may commence. The vendor shall save the City of Lebanon and all the City of Lebanon’s employees harmless from all suits, actions or claims of any character brought for or on account of damages received by any person, persons or property resulting from the operation of the vendor prosecuting the work under this agreement.
