Specifications include, but are not limited to: 1. The bidder agrees to provide the State of Iowa Department of Homeland Security and Emergency Management (HSEMD) an annual quantity of 1000 to 2000 NVLAP accredited dosimeters separated into 15 groups or series and individually packaged accordingly. TLDs will be type REMTrak Wallet Cards or equivalent. Each package shall be clearly labeled with the appropriate assignment name. Please see below distribution plan for more details on series grouping and assignment packaging. HSEMD will provide a listing update annually if needed. 2. The bidder will include an instruction sheet with each of the 15 shipments, which sets out NVLAP guidance relevant to the receipt, care, and storage of the dosimeters sent. 3. The bidder will identify each dosimeter with a serial number (“wearer number”) starting with 00001 through 2000. 4. The bidder will include a “valid wear date” range on each dosimeter. Valid wear dates range from July 1 of the current calendar year and extend until June 30 of the following calendar year. The entire order must be received by June 15th of each year. 5. The bidder will include a barcode on each dosimeter. The barcode, when scanned, will indicate at a minimum the “wearer number” of the device. 6. The bidder will punch (2) knockouts in each dosimeter to accept a clip holder to be located in the center of the top and the center of the left side of the dosimeter or provide another appropriate method to affix the dosimeter. 7. The bidder will supply two (2) control dosimeters per shipment for each of the 15 locations to assure that background radiation is accurately accounted for. 8. The bidder will customize the dosimeter badge to identify the Iowa Department of Homeland Security and Emergency Management. 9. The bidder agrees to provide the dosimeters as set forth above within fifteen (15) days of receipt of a purchase order, in current form, with at least a 6-month viability. 10. EMERGENCY CLAUSE – ADDITIONAL BADGES: Upon notice of an emergency, the bidder will supply 1,000 additional dosimeters of the same type and specifications as set forth above, within twenty-four (24) hours of receipt of notification at bidder’s officelocation, in current form, with at least a 6-month viability.
