Specifications include, but are not limited to: 1. The Contractor will develop and provide the credentialing process for all providers seeking to enter into provider agreements with the Department. The credentialing process shall include: a. developing and distributing applications, using the Department’s definition of eligible programs and credentialing criteria for providers; b. receiving all applications and re-applications for enrollment as a DCF provider; c. reviewing the applications for completeness and accuracy; d. independent verification of education, certification, and training requirements (Education, certification, or training requirements are verified through receipt of curriculum vitae submitted with the application.); e. ensure that necessary Connecticut State Police criminal record checks and DCF child protection service registry checks have been submitted and review background information contained in those submissions; f. conducting sex offender registry checks through both the Department of Justice and the State of Connecticut; g. reviewing the Federal Office of the Inspector General’s website registry of professional healthcare providers and entities excluded from participation in federal healthcare programs; h. serving as a liaison between the Department and the providers and participating as a member of the DCF Credentialing Committee; i. maintaining a database with current information on pending and credentialed providers including race/ethnicity of each credentialed individual; j. tracking the bi-annual interval for re-credentialing, and sending out the re-credentialing application to currently credentialed providers; k. prior to final approval of an after-school service application the contractor will conduct a site visit to assure the program is offered in a safe, secure setting. 2. The Contractor will administer the credentialing and quality management of and maintain a website directory for the following services: • After School Services: Clinical Support Child, Kindergarten through 7th grade, is site-based and provides a range of developmentally-appropriate enrichment, socialization and experiential learning opportunities for children and youth in a safe, structured and supervised setting. Clinical Support staff are available for youth and/or program consultation as needed. • After School Services: Clinical Support Youth, 8th grade through 12th grade, is site-based and provides a range of developmentally-appropriate enrichment, socialization and experiential learning opportunities for children and youth in a safe, structured and supervised setting. Clinical Support staff are available for youth and/or program consultation as needed. • After School Services: Traditional After School, Kindergarten through 7th grade, is site based and provides a provide a variety of child-centered activities that support positive development and complement school-day activities. • After School Services: Youth After School, 8th grade through 12th grade, is site based and provides a range of developmentally-appropriate enrichment, socialization and experiential learning opportunities for youth in a safe, structured and supervised setting. • Assessments is a service utilized when there is a concern that a DCF child or youth on an active DCF case has a mental health and/or a substance abuse issue that requires further evaluation. The assessment includes an initial face-to-face screening, additional face to face contacts with the individual and collateral contacts with family members, caretakers and other treatment providers to determine the individual’s strengths and limitations, to determine functional capacity, to identify natural supports and to develop or review an individualized service plan.
