Specifications include, but are not limited to: 1. Procure annual service for Medical Grade Oxygen, Cylinders and Storage Cabinets meeting the parameters, conditions, and mandatory requirements presented in this document. 2. Must be able to provide a local distribution and gas filling facility within Sedgwick County. This is to ensure there are no interruptions or delays in service during times of extremely high usage and demand. 3. Must be registered with the US Food and Drug Administration (FDA) and supply Medical Grade Oxygen under the current compressed Medical Gas Standards. 4. Provide a Certificate of Compliance from an independent inspector. 5. Annual service for Medical Oxygen, Cylinders and Storage Cabinets must meet or exceed specifications listed in this document. Must provide the specific cylinder sizes, types and valves specified in this document. 6. Provide consistent weekly service/deliveries as required to all locations listed in this bid. Depending on demand, deliveries may be required to be made more frequently. 7. Vendor must ensure Oxygen supplies at all locations are adequate prior to weekends and all federally observed holidays. 8. Vendor must demonstrate the ability to provide an appropriate local buffer stock of all sizes of cylinders. 9. Vendor shall not use an outside or third party deliver service for deliveries. Delivery drivers must be certified as CGMP and HAZMAT licensed drivers. 10. Provide a local, dedicated account support representative. Providing contact names, titles, qualifications, phone numbers and email addresses of lead professional personnel assigned to account responsible for the Medical Grade Oxygen. 11. A 24/7 on-call delivery service is required. Provide at least three (3) 24/7 emergency phone numbers for after hour or emergency service. Time limit from initial call to response shall not exceed two (2) hours. 12. Provide monthly invoicing for each account - Wichita Fire, Sedgwick County Fire, Derby Fire, Colwich Fire, and Emergency Medical Services (EMS) that includes all products purchased and equipment rental. 13. Discuss any current local, state or federal (i.e. FDA) violations and any ongoing litigation that may cause conflicts or affect the ability of the vendor to provide services or products.
