Specifications include, but are not limited to: The following information is provided to assist in the development of technical and cost proposals. The data is not intended to be all inclusive of the specific services required. Scheduling of services shall be between the hours of 8:00 a.m. and 4:30 p.m., Monday through Friday, or as otherwise mutually agreed upon; however, this does not mean PSH&TC expects services to be provided for the entirety of those hours or days. The average number of hours per week will vary depending on census and patient/treatment need. The Contractor shall bill for the actual time services are provided in 15-minute increments. Annual hours not to exceed 300. The Physical Therapist shall provide a range of modalities including but not limited to: • Muscle flexibility range and motion • Hot/cold packs • Evaluations and Re-Evaluations • Sensory and strength development • Manual therapy, modalities (ultrasound, e-stimulation, TENS unit), and • Gait and balance training • IPP Reports, annual assessments for individuals who live at PSH&TC. The Contractor shall attend suggested hospital orientation sessions and provide the appropriate credentialing documents as required by PSH&TC, and/or CMS. The Contractor shall provide credentialing documents annually, or as requested.
