Specifications include, but are not limited to: The City is requesting services for the design of a new culvert/headwalls Pine Island Road to include: Field investigation/Site assessment - A condition inspection of the culvert, headwall and catch basins; Review of current infrastructure in the area to include evaluation of current layout and drainage flows; Topographic Survey – perform a topographic survey of the project area. Final design plans for the replacement of the existing culvert/headwall – prepare final design plans for the proposed improvements. To include Specifications - Special Provisions utilizing CTDOT form 818 Opinion of Probable Cost; Plan revisions based on City comment/input; Construction Documents – prepare bid documents for advertisement by the City. Develop bid forms for contractor pricing and furnish final design plans and special provisions along with front-end document to the City; Determine appropriate permitting necessary – to include pre-application meeting with Department of Energy and Environmental Protection and any other regulatory agencies to determine or verify the type of permit and extent of application materials required; Assist the City in applying for any and all necessary permits (i.e., DEEP Certificate of Permission, DEEP General Permit, USACE permitting) – Prepare application and supporting documentation including technical reports, response letters, and coordination letters normally required as part of the permit application process.
