Specifications include, but are not limited to: (ME) ADMISSION SCREENING The Contractor shall conduct a nutritional assessment on newly admitted Home residents within five (5) days of the resident’s admission date. The Contractor must complete Section K of the minimum data set (MDS) assessment and a Care Area Assessment (CAA) computer data entry within the timeframe set forth by the MDS Coordinator. The Contractor shall submit a care plan for the resident to the Home within fourteen (14) days of the resident’s admission. The Home must notify the Contractor of the admission and provide resident information to the Contractor by facsimile. (ME) MONITORING AND REVIEWS The Contractor shall monitor all residents determined by the Contractor to be at nutritional risk and those residents receiving enteral feedings at least monthly. The Contractor shall review all residents and complete a nutrition assessment for each resident at least quarterly. The Contractor must also complete full comprehensive Nutritional Assessment and complete the computer data entry within the time frame set forth by the MDS Coordinator upon significant change of status and at least once per twelve (12) month period for each resident. The Contractor must recommend to the Home nutritional pharmaceuticals for residents with specific nutritional needs. (ME) DIETARY AND HYDRATION ASSESSMENTS The Contractor must provide Dietary and Hydrations Assessments per Centers for Medicare and Medicaid Services (CMS) regulations and Idaho Dietary Manual, including computer data entry, within the timeframe set forth by the MDS Coordinator requested by the Home and the resident's physician. The contractor must document assessments in the resident's chart within seven (7) calendar days of the completion of an assessment.
