Specifications include, but are not limited to: 1. A public annual report on or before October 15 for the period of July 1-June 30 of each year, to include, at a minimum: a. An overview of an annual strategic plan and specific goals b. Specific activities and accomplishments , including, but not limited to, the following details: i. MCHD’s role in or contribution to the activities and accomplishments ii. Quantifiable measures and impacts to the Multnomah County residents for each activity and accomplishment iii. Impact of each activity or accomplishment on Health Equity, including the quantitative reduction of health disparities on Multnomah County residents with a specific focus on BIPOC communities c. List of member clinics d. List of community partners e. Board members and meeting schedule f. Demographics of member clinic clients served, including but not limited to: i. Race and ethnicity ii. Gender iii. Income level (by Federal Poverty Level (FPL)) 2. An annual financial report including, at a minimum a. Revenue sources and expenditures b. Dues and other financial or in-kind contributions from each member clinic c. Demonstrates that MCHD funds were not used for activities prohibited by Health Resources and Services Administration (HRSA) grant requirements (https://www.hrsa.gov/sites/default/files/hrsa/grants/manage/legislative-mandates-grantsmanagement-2023.pdf) 3. A quarterly report to be submitted along with each invoice showing, at a minimum: a. Costs and description of how MCHD dollars were allocated, including: i. Staff time and salary ii. Direct cash support, technology, supplies, services, and insurance purchased, which member clinics received these items iii. Brief narrative of how the expense improves the health of the residents of Multnomah County b. Updates (frequency agreed upon by both parties) on demographics of member clinic clients served at FQHC, FQHC Look-alikes, or clinics that serve more than 10,000 patients per year, including but not limited to: i. Race and ethnicity ii. Gender iii. Income level (by Federal Poverty Level (FPL)
