Specifications include, but are not limited to: A. Provide culturally responsive outreach, education, and engagement for Black and African American or Hispanic and Latino/a/x/e communities, with a focus on reaching older adults and increasing access to information, and referrals to resources and benefits related to aging in the community. B. Address barriers to accessing benefits or helping with system navigation. C. Provide onsite or virtual support as it makes sense (due to geography or cultural focus) D. Work with County to develop a direct/warm hand-off for DAVS services, with a particular focus on ADRC, Older Americans Act and Oregon Project Independence programs. E. Develop and share messaging and materials or media to reach Black and African American or Hispanic and Latino/a/x/e communities, including online or social media and printed materials. F. Address social determinants of health or support other community needs as needed to build trust and relationship. G. Advise and collaborate with Washington County efforts to reach, build trust with, and better serve Black and African American or Hispanic and Latino Communities.
