Specifications include, but are not limited to: Provide outpatient facilities, equipment and locations to serve clients who reside in Utah that have sustained a Spinal Cord Injury (SCI) and/or Brain Injury (BI); Identify the number of anticipated SCI and TBI participants to be served; Identify an underserved population that will be targeted and number of participants served; Homeless population Rural areas Different minority populations (i.e. Native Americans, Pacific Islanders, Hispanic, etc.) Skilled nursing facilities- SNF Long-term acute care Intermediate Care Facilities- ICF Provide a plan with how services will be provided to participants to the identified underserved population. Provide transportation and/or housing as appropriate to participants. Provide a plan as to how a person will qualify for the transportation and/or housing Staffing plan for serving the number of anticipated participants of the Fund; Implement a recruitment process to identify and enroll participants; Provide intake assessment of clients to identify if they qualify for the fund. To qualify: Have a spinal cord injury (SCI) or brain injury (BI) that is non-progressive non-degenerative; Client must be a resident of Utah. They must have a valid Utah identification card, valid driver’s license, or a current utility bill showing address; and Have no other means to pay for services. Provide the following post-acute-care services, where applicable, according to the rate 100% of Medicare per approved CPT code or a set rate determined by the Spinal Cord and Brain Injury Rehabilitation Fund Advisory Committee. Physical therapy; Occupational therapy; Speech therapy; Extended Sustainable Rehabilitation (ESR). ESR is a type of wellness program. Driving evaluation and/ or Driving Training. Prior approval is required from the Spinal Cord and Brain Injury Rehabilitation Fund Advisory Committee for any requests to purchase needed program specific rehabilitation equipment purchased via the SCI/BI Fund. Provide at least a part time employee to assist with administrative services of the Fund that is paid for by the Fund Track individual progress and evaluate using an approved assessment. Track and measure overall success for this project including where applicable: a) Improvement in quality of life; b) Improvement in transfer skills; c) Improvement in ambulation; d) Improvement in wheelchair mobility; e) Improvement in cognitive skills; f) Participate in recreational and leisure activities; g) Return to school or work; h) Participate in the ESR; i) Number of re-hospitalizations; and j) In-kind services.
