Specifications include, but are not limited to: Through this Request for Proposal (RFP) the Agency of Human Services/Vermont Department of Health (hereinafter the “State”) is seeking to establish contracts with one or more companies that can provide a You First Client Data System. The You First program currently serves approximately 650 Vermonters by covering the cost of breast & cervical cancer screenings, diagnostic follow-up, healthcare system navigation and healthy lifestyle perks. These services are available to members 21 and older that meet income eligibility requirements as put forth by the CDC via the National Breast and Cervical Cancer Early Detection Program (NBCCEDP) and Well-Integrated Screening and Evaluation of WOMen Across the Nation (WISEWOMAN) initiative. In addition to collecting and reporting the MDEs, any system must also support the daily workflow of the You First program which includes: • Managing member enrollment including issuance of membership cards • Facilitating screening outreach including generating reminders and tracking results • Facilitating patient navigation and LifeStyle Interventions (LSI's), and tracking member progress • Maintaining information about contracted providers of services • Tracking billing information and matching this to services provided The program is currently meeting these needs via the MedIT system. Any solution must include transfer of legacy data from this system.
