Specifications include, but are not limited to: The Lane County Public Health’s Healthy Families Lane County (HFLC) program is seeking proposers to provide comprehensive home visiting services using the Healthy Families America & Maternal Infant Early Childhood Home Visiting (MIECHV) models for expectant and new parents of newborns living in Lane County. Family Support Specialists (FSS) meet with families primarily in the homes of the families served. The outcome of each model (model description follow under “Program History”) is to link families to community activities and supports to reduce isolation, support social integration, preserve individual and/or family cultures, improve health and educational outcomes, and broaden both natural and system supports. While services can be offered to any eligible family, we prioritize services to populations experiencing pregnancy-related healthcare disparities; see attached glossary of terms and below narrative on priority populations. (Glossary is contained in Attachment 2, the PPPM) Through this RFP process, the County has a strong preference to award 1 contract for 1 Home Visiting Team (HVT) at a community-based organization located within Lane County. Lane County Public Health reserves the right to modify the number of contracts awarded or fund partial HVTs through this RFP.
