Specifications include, but are not limited to: 3.1.1.1 Improve Outcome-Based, Person- and Family-Centric Model of Practice Rooted in Family Engagement and Empowerment Historically, each DHS program has provided benefits and services to the children and families it serves independently of each other, driven by program-specific goals and mandates. DHS systems were similarly siloed and do not easily share information across Divisions regarding the potential needs of the children and families or the services received from other DHS programs. DHS is moving to a more person- and family-centric approach focused on services to improve access and outcomes, reduce cost, and raise the quality of DHS programs, and services. 3.1.1.2 Leverage DHS’s MDM, Data Exchanges, and Interfaces DHS has aligned with federal requirements and aims to achieve a technology solution that supports the Administration for Children and Families’ (ACF) CCWIS goals and mandates. The CCWIS Solution shall allow the Department to support secure data exchanges, data interfaces, and shared services and components. 3.1.1.3 Improve Safety, Permanency and Well-being through Enhanced Service Delivery The safety, permanency, and well-being of children and families is a top priority for CWSB. CWSB enhanced its service delivery by creating a Crisis Response Team (CRT), initiating prevention services such as evidence-based intensive home-based services, and home visiting services. Reunification is the primary permanency goal for all children placed in foster care. Tracking and monitoring progress and outcomes, including the length of stay in foster care is critical to ensure timely reunification. The CCWIS Solution shall electronically track and monitor outcomes in real time.
