Specifications include, but are not limited to: Through this Request for Proposal (RFP) the Office of Purchasing & Contracting (hereinafter the “State”) is seeking to establish contracts with one or more companies that can provide Cremation Services and Grave Markers for the Department of Corrections. 2.1.1.1. Contractor shall respond within 1 business day to a call from the Department of Corrections to schedule service. Contractor shall provide a reliable phone number for afterhours calls. 2.1.1.2. Bidders shall provide proof of licensure to perform cremation services with their bid. 2.1.1.3. Contractor shall not release any information about or discuss a death, death scene or decedent to any unauthorized persons. All information regarding decedents, investigations, or other matters shall be held in the strictest confidence at all times. 2.1.1.4. Removal 2.1.1.4.1. Retrieval of the decedent’s body from its current location. Contractor personnel responding to the request must be licensed or authorized to perform the required services. 2.1.1.4.2. Transportation of the decedent’s body in a suitable vehicle from pick-up location to the Contractor’s facility in accordance with accepted industry practices. 2.1.1.4.2.1. Out of state transport fee shall be quoted by the Contractor at the time arrangements are made. 2.1.1.4.3. Meet stated response time for retrieving the body. 2.1.1.4.4. Bidders shall indicate on the price schedule which counties they can provide service to. 2.1.1.5. Storage and Cremation 2.1.1.5.1. Refrigeration and/or embalming of decedents for as long as necessary. 2.1.1.5.2. Cremation of the remains of the decedent at a licensed crematorium. 2.1.1.5.3. Provision of a basic urn or other approved container for storage of cremains. 2.1.1.5.4. Delivery of cremains to the designated recipient within 30 days unless otherwise specified by the Department.
