Specifications include, but are not limited to: Provide leadership by facilitating 4 (four) meetings among community agencies and partners providing after the fact (e.g., suicide) education for families and children, peer-to-peer support group for children from 11 to 17-years-old attending Kalihi Elementary School, Leilehua High School, Pearl City High School, after school programs, clinics, with focus on child injury prevention and health education. Be able to purchase incentives with focus on kids’ injury prevention. This may include home safety devices (e.g., toilet seat locks, doorknob locks, gun locks for families who have guns in homes, car seats, life vests, bike helmets, pinwheels, thermometers, first aid kits, etc.) Bidder shall be responsible for purchasing and tracking items that meet quality/national safety standards. Bidder shall establish an inventory and delivery method to ensure items are evenly distributed. Bidder shall provide storage for the items purchased. Be able to hire a notetaker for the Hawaii Child Death Review Committee Bidder shall manage and provide payment for invoices and process reimbursements for 2 (two) persons related to 1 (one) out-of-state national conference (e.g., City Match, or other) where there are opportunities for information on children prevention and implementation discussions and sessions as directed by designated MCHB Program staff. Performance management, evaluation, and deliverables- bidder shall provide 2 reports, first one after 6 months of the procurement period and second one by the end of the procurement period to the DOH/MCHB summarizing community served, health education and incentives provided, and project achievements during the reporting period. Project sustainability- bidder shall deliver a report by the end of the procurement period with strategize ways to sustain project to ensure health education and child injury prevention initiatives in the future.
