Specifications include, but are not limited to: Consultative and contract services required to implement and test Delawares State Healthcare Innovation Plan including (1) strategic planning, (2) project management, (3) stakeholder support and engagement, (4) research and analysis including, but not limited to, predictive and actuarial modeling, economic analysis and (5) all process and outcome evaluation and reporting in the following areas: a. Health care delivery system analysis and transformation including implementation and testing of new delivery system models impacting at least 80% of Delawares population b. Population-based approaches to health promotion and disease prevention statewide c. Health care payment model transformation including implementation and testing of new multi-payer, value-based models d. Health care quality and cost data collection and analytics including implementation of statewide shared utilities to support delivery and payment models and consumer transparency e. Health care workforce transformation to support multi-disciplinary, teambased, patient-centered approaches to support new delivery and payment models f. Health policy research, development, analysis and evaluation to support statewide transformation and innovation
