Specifications include, but are not limited to: The North Dakota Department of Health is seeking a Worksite Wellness Consultant with experience to provide activities, which targets worksites. This is a pilot project. We are seeking a Worksite Wellness Consultant to provide the following: Travel required to worksites in southeast North Dakota (includes, but not limited to the cities of Fargo, Jamestown, Valley City, Wahpeton, Lisbon). This may expand to other cities within North Dakota. Create a worksite assessment tool. Recruit worksites to participate in the project. Implement the assessment tool with each worksite and provide recommendations for improvement to each worksite based on the assessment results. Provide a monthly progress report of work activities with invoice. Provide a final report by July 20.
