Specifications include, but are not limited to: ND Workforce Safety & Insurance is soliciting proposals for Customer and Employee Satisfaction Survey services. The Department wants assistance in obtaining feedback from our internal and external customers (injured workers, employers, medical providers, and employees) regarding the quality of services provided by Workforce Safety & Insurance (WSI). Workforce Safety & Insurance will provide lists of injured workers to be contacted for all telephonic surveys. WSI will do all the printing and mailing of the paper surveys to our employers and medical providers. WSI will also provide a list of all employee email addresses for use during the employee satisfaction survey. Contractor will furnish postage paid, business reply envelopes (size #9), containing their mailing address, to Workforce Safety & Insurance to be included with the paper surveys (employer and medical providers). The employer and medical provider surveys will be mailed by WSI. Contractor may bill WSI for the cost of these envelopes and must include these costs in the cost proposal of this RFP. Contractor is responsible for any software licenses and costs used in the creation of online survey tools. Contractors will be accountable for the accuracy of survey data as well as timely completion of the written reports. Contractor will be expected to provide revised copies of written reports if any inaccuracies are identified by WSI. Incidents of inaccuracies and untimely reporting will be included in the service evaluation conducted at the end of the contract period.
