Specifications include, but are not limited to: The Department of Social Services Division of Behavioral Health is soliciting proposals from qualified private entities to provide community coordinator services to assist in the management of South Dakota Screening, Brief Intervention and Referral to Treatment, a federal grant awarded to the State of South Dakota. The Offeror will be responsible for acting as a liaison between the medical clinics and the community based provider network, preparing agendas and minutes for all meetings, assist with planning of trainings and the development of workflow documents, assist with data collection activities, and work with the program manager on the preparation of all required federal reports.
