Specifications include, but are not limited to: The State of Delaware, Department of Health and Social Services (DHSS), Division of Public Health (DPH), is seeking proposals from potential vendors to work with nineteen school districts to operate thirty-two School Based Health Centers (SBHCs). Potential vendors must bid based on all high schools within the district as listed in the Availability of Funds section (examples, Colonial School District – William Penn High School, Christina School District – Christiana, Glasgow and Newark High Schools). The overall mission of the school-based health center initiative is to provide preventionoriented multidisciplinary health care to adolescents in their public school setting. These services are coordinated with the medical, health education and pupil personnel services currently being provided by school districts and with the student’s medical home. The contractor will be responsible for establishing and operating a SBHC(s) Proposals shall address the following: 1. Organizational capacity and qualifications; 2. The projected number of students to be served and the percentage of the student population they represent; 3. An operational plan to include staffing, hours of operation, a schedule of services; and a description of how the physical location will meet operational needs
