PERFORMANCE WORK STATEMENT B38 Canteen Commercial Kitchen Flooring Replacement INTRODUCTION/BACKGROUND: Tuscaloosa Veterans Affairs Medical Center (TVAMC) located at 3701 Loop Road East, Tuscaloosa, Alabama 35404 requires a service contract for flooring replacement in Building 38, Canteen Services, Commercial Kitchen and Canteen Services basement elevator lobby. B38 Canteen Commercial Kitchen is 1200 sq ft of quarry tile outfitted with commercial appliances, sinks, refrigerators, and freezers. The Basement elevator lobby area is an additional 250 sq ft. OBJECTIVES: The Contractor shall furnish all contract oversight, management; supervisory and technically trained personnel; uniforms; equipment, materials, and supplies necessary to remove existing Kitchen tile flooring while leaving the integrated wall base in place and install new tile as specified below. Modifications and replacements are to address the disrepair of the existing flooring. MAJOR REQUIREMENTS: For the Contractor to accomplish the work under this purchase order, it shall be necessary to complete the following tasks: The Contractor shall supply all materials, supervision, and labor to remove and store existing equipment from the commercial kitchen. The Contractor shall supply all materials, supervision, and labor to remove existing tile flooring while leaving the integrated wall base in place and unharmed. The Contractor shall supply all materials, supervision, and labor to provide sub-floor preparation before new flooring installation begins. The Contractor shall supply all materials, supervision, and labor to install new Canteen Kitchen flooring: Crossville, Cross-Colors Mingles, Cross-Tread, Color Graphite A850 CTS, 8x8. Installation Pattern: Vertical Ashlar per manufacturers guidelines using the adhesive/mortar/grout also detailed in manufacturers specifications guide. The Contractor shall supply all materials, supervision, and labor to install new Canteen Basement elevator lobby flooring with integrated cove base: Crossville, Cross-Colors Mingles, Cross-Tread, Color Graphite A850 CTS, 8x8 with 6x8 cove base. Installation Pattern: Vertical Ashlar per manufacturers guidelines using the adhesive/mortar/grout also detailed in manufacturers specifications guide. The Contractor shall supply all materials, supervision, and labor to seal floor penetrations after installation of new floor. The Contractor is responsible for providing dumpsters for removal of all trash and debris associated with this contract. The Contractor shall supply all materials, supervision, and labor to reinstall all kitchen equipment back to its original location and ensure operation. The Contractor shall supply all materials, supervision, and labor to provide a deep final clean before the space is turned back over to Canteen Services. REPORTS/DELIVERABLES: Contractor to perform a pre-walkthrough assessment with the COR. Contractor to complete major requirements listed above. Contractor to perform a final walkthrough assessment with the COR. PERIOD OF PERFORMANCE: This contract Period of Performance will be 90 days from date of award. GENERAL REQUIREMENTS: The Contractor is responsible for ensuring that its company and staff maintain any generally required professional certification, accreditation, license, bond, and proficiency relative to their area of expertise. The Contractor shall provide the TVAMC documentation of such records and updates throughout the Contractual Period. The Government will not pay expenses to meet these requirements. The on-site supervisor is responsible for management and oversight of all crew members. To be eligible to perform under this contract, each crew member shall meet the following: The ability to meet and deal with the public in a friendly and customer service-oriented manner. The ability to read, understand and apply printed rules, detailed orders, instructions, and layouts in English. Expected to be physically able to perform the functions required in the performance of their assigned duties, such as but not limited to, lifting, bending, squatting, and standing for excessive periods of time. Maintain a neat, clean appearance and in proper uniform at all times. Proper uniform should include shirt with company name and or logo, long pants, and steel-toed shoes. The onsite supervisor is required to obtain/maintain a 30-hour OSHA card and all other labor crew must obtain/maintain a 10-hour OSHA card at no additional expense to the TVAMC. In the event of a staff change, the new employee must complete and submit certification before working in the facility. The Contractor and all employees working at the TVAMC are required to comply with CM 11-30, Tuberculosis Control Plan, policy, and procedure. Workers are to be advised that proof of TB Skin Testing is a condition of their employment, including obtaining required tuberculin skin tests (TSTs) and follow-up, as appropriate; attending educational sessions; and using safe work practices to prevent exposure to Mycobacterium TB. Workers may consult the Infection Control Preventionist (ICP) for questions and concerns regarding TB Skin testing. The Contractor will be responsible for ensuring compliance by its employees with the security regulations of the Veteran s Administration where work is performed under this Contract. COR will print and review va10176 VA Privacy and Information Security Awareness and Rules of Behavior and VA Privacy and HIPPA Training 10203 with all contractors on station for this contract. Contractors will sign certifications of completion for the Contracting Officer Representative (COR) to maintain in the contract file. All contractors that come on site will have to be issued a Non-PIV badge. There will be certain circumstances where we will have contractors working in WHEN hours and will not be able to come during the week. We will work with the PIV office in those circumstances to ensure that we can figure out the most appropriate course of action. Please collect the following information from every contractor on campus that does not already have a VA approved PIV badge: Legal Name Dates of work (please be generous with this, many times we have contractors come back for warranty work) 2 forms of ID per Table 1 below. Do not send anything with a SSN number on it. Once you have collected this data you will send the PIV office a list of contractors and their information. We will schedule a time with him to start badging all contractors currently on campus, preferably 48 hours beforehand. The PIV office will fingerprint each contractor and perform a Special Agreement Check (SAC), a type of quick background investigation. As long as the information on the documents matches up with each other and the SAC comes back clean, badges should be able to be printed for the contractors that day. This will be the process for all new arrivals also. No unauthorized Photography or Video on medical center premises. Per VA Regulations 1.218(b) (23) Use of recording devices of any kind to record patients or employees is strictly prohibited. Upon completion or termination of the applicable contract(s) or agreement(s), return and/or destroy, at Covered Entity s option, VA information gathered, created, received, or processed during the performance of the contract(s) or agreement(s). No data will be retained by Business Associate, or contractor, subcontractor, or other agent of Business Associate, unless retention is required by law and specifically permitted by Covered Entity. As deemed appropriate by and under the direction of Covered Entity, Business Associate shall provide written assurance that all VA information has been returned to Covered Entity or destroyed by Business Associate. If immediate return or destruction of all data is not possible, Business Associate shall notify Covered Entity and assure that all VA information retained will be safeguarded to prevent unauthorized Uses or Disclosures. Prior to termination or completion of this contract, contractor/subcontractor must not destroy information received from VA, or gathered/created by the contractor while performing this contract without prior written approval by the VA. Any data destruction done on behalf of VA by a contractor/subcontractor must be done in accordance with National Archives and Records Administration (NARA) requirements as outlined in VA Directive 6300, Records and Information Management and its Handbook 6300.1 Records Management Procedures, applicable VA Records Control Schedules, and VA Handbook 6500.1, Electronic Media Sanitization. Self-certification by the contractor that the data destruction requirements above have been met must be sent to the VA Contracting Officer within 30 days of termination of the contract. INTERIOR WORK STANDARDS The Contractor is to provide all supervision, labor, equipment, tools, supplies and materials to provide the complete interior painting for the project as per directions provided by the COR. The Contractor confirms that he is an expert in this field of work and is fully knowledgeable and experienced in all aspects of procedures, methods, regulations, codes and requirements and the Contractor further acknowledges that the TVAMC is relying on this expertise. Concept plans will be designed by the COR and given to the contractors to use as a guide for the work needing completion that day/week Concept plans will provide product specifications with installation layout being requested for each area. The Contractor is responsible to protect other finish products in the interior of the building while performing his work The Contractor will provide dust barrier at entry/exit door of work area. The Contractor will provide walk off mat in corridor or space outside of work area. CONTRACT WORK HOURS: The contract normal work hours will be 8:00am to 4:30pm Monday- Friday excluding Government holidays. The Government has the option to modify the normal work week, days and hours, as necessary for the accomplishment of TVAMC mission. WHEN (weekend, holiday, evening and nights) hours may be needed to accomplish a job, contractors will need to be available. 9. QUALITY CONTROL: The Contractor shall specify personnel that will serve as a single point of contact for all Quality Control issues. This person must be approved by the COR.
