Vendor shall be an OEM, authorized dealer, authorized distributor, or authorized reseller for the proposed equipment/system, verified by an authorization letter or other documents from the OEM. 5. A completed 52.225-2 Buy American Certificate. 6. Proposals shall clearly articulate how the vendor s product fulfills the requirements listed in section B.2 Statement of Work and providing a completed attachment D.1 Evaluated Salient Characteristics spreadsheet. 7. Completed copy of VA Form 6550 Pre-Procurement Assessment. 8. Specification/cut sheets for any/all network and hardware components. 9. The Contractor shall adhere to VA national site-to-site VPN or work with the VA Office of Cyber and Information Security and VISN 23 Information System Security Officers to establish a client-based VPN. 10. Information about the company s support structure during the warranty period (i.e. a listing of Field Service Engineer locations and availability, support 800 phone number(s), remote support, emergency response and parts timeframe, etc.). 11. A description of additional training offered by your company. 12. A list of additional features and functions that could be added to the system beyond what is required. 13. The Contractor shall provide a pre-recorded live demonstration of the video integration system, not longer than one (1) hour long, to describe how the video integration system would perform in a clinical setting.
